Oculoplastic Surgery is a subspecialty of Ophthalmology that concentrates on the eyelids, tear drainage system and orbits (bones and tissues around and behind the eyes).

If you wish, our staff will send insurance and new patient questionnaires. Please complete the forms and bring them on the day of your consultation. Also, provide the name, address, telephone and fax numbers of your primary care physician's office.

Please provide a clearly printed list of any drug allergies and all of your medications with their doses and frequency of use.

If you are a contact lens wearer, it would be best if you would wear your glasses. If you decide to wear contact lenses please bring your glasses and a contact lens case.

If the referring problem required a recent CT or MRI scan, please bring the actual scan and the radiology report with you on the day of your consultation.
